The Center for Interprofessional Education and Collaborative Care is offering Caring For Complex Patients, a special learning experience for health sciences students in Fall 2017 and Spring 2018.
By participating in this experience, you contribute directly to improved health outcomes of patients in your own community; enhance your professional practice; and provide opportunities for personal scholarship.
With faculty support, your interprofessional student team will engage with two complex patients in a variety of settings (e.g.: home, community, primary care, or hospital). Student team may provide care coordination support, including: finding and connecting patient(s) to community resources, and providing a soft handoff back to the patient’s primary care at the end of the experience.
